My name is Dr. Rayna Kamerzell DC . I am the Medical Director at Raw Center for Strength. I have been fortunate to work with some of the finest sports trainers in the industry, some of whom also happen to be Chiropractors. Although I cannot speak for an entire profession, I feel comfortable discussing my views of manipulation.

First of all, the "bone out of place" approach is dead.Manipulation does not move bones. Manipulation restores motion. Spinal manipulation is one of the most researched and proven to be one of the most effective interventions for mechanical back pain. It seems to be more effectiveif combined with corrective exercise and activity modification/ movement pattern retraining.

There are a number mechanisms that contribute to the effectiveness of a spinal manipulation. Post manipulation there is a reflex relaxation of surrounding musculature, as seen clinically and confirmed on EMG. There appears to be a short term central anesthetizing effect after manipulation that decreases pain perception. In joints that are surrounded with fibrotic scar tissue, the manipulation can break up adhesions and allow proper motion.

There are times when mobilization of a joint is necessary to restore proper function. Look at the now famous joint by joint approach. Stability and mobility are functions of each other. Loss of motion in one joint creates more motion in the joint above or below as a compensation. This happens in the spine as well. One of the most difficult parts of being a manual therapy clinician is determining when a joint needs mobility, which at times can be achieved easily with a manipulation, or stability which is gained through proper rehabilitation. A good manual therapist is able to look at the big picture of the patient's functional movement and make an appropriate clinical decision. Unfortunately, many do not, and my profession gets a bad rap.

Over the last of year I have had the chance to reinvent my ideas on joint function. Stability is important, but I also understand that there are times when you need to establish mobility in order to build stability. I feel that the best chiropractors are the ones that understand retraining movement patterns as part of the treatment and rehabilitation process.
